Social Model Day Care are community-based programs offering socialization, personal care, supervision, recreation, and
a nutritious meal in a protective setting to those frail or impaired elderly adults who cannot remain alone, but do not require
24-hour residential care. Eligibility, fees and services provided vary. Programs are supported by federal, state, county and
town funds, and private funding.
